Description

The Massachusetts edition is purpose-built for the recordkeeping a DPH, DDS, or MAP inspector expects to see at your facility. Every page references the Massachusetts statute or regulation it satisfies, so your nursing staff knows exactly why a field exists — and your inspector finds what they're looking for.

Designed in collaboration with charge nurses and group-home administrators across the Commonwealth, the layout puts the most-used fields (date, time, patient, medication, dose, witness, count remaining) at the top of every entry and reserves the bottom of the page for end-of-shift reconciliation.

FAQ

Is this MAP-approved?

The book is designed to support MAP recordkeeping requirements but is published independently of the Department of Public Health. MAP approval is a facility-level certification, not a stationery one.

How many entries per book?

Each daily log page holds 12 entries. Standard stock book contains 240 log pages — roughly 30 days of high-volume use or 90 days for a typical 8-bed group home.

Can I get custom-branded copies?

Yes — orders of 25+ qualify for custom cover branding. Contact us for a quote.
